Some errors are down to mistakes by individual doctors. Some are due to inexperience. Others are system failures - administrative errors in failing to call back a patient for review or perhaps abnormal test results were simply filed instead of being acted on. No one sets out to make a mistake, but they continue to happen in both the NHS and the private sector.

- the benefits of early treatment can be lost
- sometimes treatment is given for the wrong condition
- inappropriate drug therapy or surgery can cause damage quite separate and in addition to what was really wrong in the first place
